QABA’s accommodations policy outlines the guidelines and procedures for providing reasonable accommodations to individuals with disabilities or specific needs.   2. Definition of Accommodations: The online exam accommodations form provides information from your doctor to access and participate fully in QABA’s enrollment, examination, and renewal.
  3. Request Process: Individuals may request accommodations by completing the Accommodation Form along with providing supporting documents, to our online application. Please allow 30 days for QABA to review and approve your accommodation request before scheduling your exam.
  4. Evaluation and Approval Process: Once your application is submitted, QABA will review the information and make a decision. QABA reviews the submitted documentation to assess the validity of the request and determine if the individual meets the eligibility criteria for accommodation. This may involve verifying medical or disability-related information.It is important to note that each accommodation request is an individualized, interactive process to assess the request and determine reasonable accommodations.
